1. Your Protected Health Information (PHI)
“Protected Health Information” (PHI) refers to any information that identifies you and relates to your:
- past, present, or future physical or mental health or condition;
- healthcare treatment provided to you; or
- payment for healthcare services.
This includes any information you provide during consultations, evaluations, appointments, medical forms, or online submissions.
2. Our Responsibilities
BarX Men Health Clinic is legally required to:
- maintain the privacy and confidentiality of your PHI;
- provide you with this Notice explaining our legal duties and privacy practices;
- notify you if a breach of your unsecured PHI occurs;
- follow the terms of this Notice currently in effect.
3. How We May Use and Disclose Your PHI
We may use and disclose your PHI without your written authorization for the following purposes:
A. Treatment
To provide, coordinate, or manage your healthcare and related services. This may include communication with licensed physicians, nurse practitioners, specialists, or other healthcare professionals involved in your care.
B. Payment
To bill and collect payment for services provided to you. This may involve contacting you, your insurance provider, or other entities responsible for payment.
C. Healthcare Operations
For internal operations such as quality assessment, training, audits, compliance reviews, and general clinic management to ensure high standards of care.
4. Other Uses and Disclosures Permitted by Law
We may also use or disclose your PHI without your authorization when required or permitted by law, including:
- public health reporting and disease prevention;
- reporting abuse, neglect, or domestic violence as required by law;
- health oversight activities and regulatory audits;
- responses to court orders, subpoenas, or legal proceedings;
- to prevent or reduce a serious threat to your health or safety or that of others.
5. Uses and Disclosures Requiring Your Authorization
In all other situations, we will ask for your explicit written authorization before using or disclosing your PHI. This includes:
- marketing purposes;
- third-party data sharing;
- use of images or testimonials;
- psychotherapy notes (if applicable).

You may request to inspect or obtain a copy of your medical records in paper or electronic format.
B. Right to Request Amendments
You may request corrections to your PHI.
C. Right to Request Restrictions
You may request restrictions on certain uses of your PHI.
D. Right to Confidential Communications
You may request communication through specific channels.
E. Right to an Accounting of Disclosures
You may request a list of PHI disclosures made by our clinic.
F. Right to File a Complaint
You may file a complaint with us or the U.S. Department of Health and Human Services (HHS).
