This Telemedicine Consent explains how telemedicine services are provided by BarX Men Health Clinic, how your medical information may be used during remote care, and your rights as a patient. By proceeding with telemedicine services, you acknowledge that you understand and agree to the terms below.

1. What Is Telemedicine?

Telemedicine involves the use of electronic communications—such as video conferencing, phone calls, secure messaging, digital forms, and the transmission of medical data—to deliver healthcare services remotely when you and your provider are not in the same physical location.

2. Possible Benefits of Telemedicine

• Improved access to medical care
• Convenient scheduling and reduced travel time
• Continuity of care and follow-up
• Access to licensed specialists

3. Possible Risks of Telemedicine

While telemedicine is generally safe and effective, potential risks include:

• Technical issues such as poor internet connection, audio or video failure
• Delays in evaluation due to incomplete information
• Potential security risks despite strong encryption and safeguards
• Limited ability to perform a full physical examination

Your provider may determine that an in-person visit is necessary for accurate diagnosis or treatment.

4. Patient Acknowledgment and Consent

By participating in telemedicine, you confirm that you:

• authorize the use of electronic communications for your medical care;

Your telemedicine visit is protected under HIPAA, California privacy regulations, and BarX Men’s Privacy Policy and Notice of Privacy Practices. We use encrypted platforms and reasonable safeguards to protect your information. However, no system is 100% risk-free, and data breaches—though unlikely—may occur.

6. Your Rights as a Telemedicine Patient

You have the right to:

• withdraw your consent to telemedicine at any time;
• request an in-person appointment whenever reasonably available;
• ask questions about your treatment before, during, and after your visit;
• access and request corrections to your medical records;
• file a complaint without fear of retaliation.

7. Emergencies

Telemedicine is not intended for emergency medical situations. If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room.

Telemedicine services at BarX Men Health Clinic are provided by licensed healthcare professionals in the State of California. Medical oversight is provided by:

If you do not agree with these terms, please notify us immediately and do not proceed with telemedicine appointments.
